All wholesale customers are required to complete a State of Hawaii — Department of Taxation Resale Certificate for Goods Form (Form G-17). Please send the completed form by email, fax or regular mail. Once the form is received and reviewed, we will contact you regarding your wholesale status. Please note that wholesale orders are not eligible for $5 Flat-Rate Shipping. Standard shipping rate apply.
